The Preparticipation Physical Evaluation (PPE), commonly referred to as the "sports physical form," has been updated as of July 1, 2024. The sports physical form has been tailored to comply with New Jersey law and has been adopted by the New Jersey Department of Education (NJDOE)

There has been a change in the requirements for submission of the updated Preparticipation Physical Evaluation. The History Form and the Physical Examination Form within the package DO NOT need to be submitted to the student's school nurse. The History Form and the Physical Examination Form should be kept by the health care provider who completes the exam. 

The ONLY FORM required for sports participation from the physician is the "Medical Eligibility Form"  and it should be submitted to the school nurse.

Here is the Medical Eligibility Form for sports participation. 

A  Health History Update Questionnaire is needed for each NEW sport in a school year if the Preparticipation Physical Evaluation is current within the year. 

If a student's Preparticipation Physical Evaluation was completed over 90 days prior to the first day of official practice in an athletic season, a Health History Update Questionnaire is still required.
